Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

We accept all major credit cards (Visa, Mastercard, American Express, Discover). We also accept cash, checks and Care Credit.

Care Credit is a third party financing option to help patients pay for dental treatment that would otherwise be difficult to afford, breaking the payment up into manageable monthly installments. Our Administrative team is happy to assist patients in exploring this option further and getting them set up.

Our office offers patients that have no other dental benefit plan in-house financing. Contact us to learn more about what plan you can fit into and the money you can save, we strive to make your oral health and dream smile a goal you can attain.

Dental Insurance

Our office partners with Delta Dental Premier as in network providers.

We will gladly bill other forms of dental insurance, however we would not be an in-network provider which may leave a portion of the treatment as the responsibility of the patient. Please feel free to contact our office with any questions regarding your personal dental benefits.
